Risking it all for the Lashes

Having long, thick, and beautiful lashes is very important to most people who wear makeup. Many apply mascara without even looking in the mirror or while in a moving vehicle, and this can cause horrible accidents – such as getting a corneal abrasion! One of the most common eye injuries is getting poked in the eye by a mascara wand. This takes some time to heal and can be painful. Aside from a possible corneal abrasion, you may run a risk of a serious infection from bacteria getting into your eye, especially if your mascara is not replaced every three months. And if you are sharing your mascara with others (which is not recommended), it should be replaced more frequently.

False eyelashes and eyelash extensions look beautiful and definitely make your eyes pop, but sometimes the glue used to apply the false eyelashes can enter your tear film which can increase symptoms of dry eye, inflammation, blockage of the lacrimal drainage system, cause corneal or conjunctival abrasions, and/or a bacterial infection. When getting eyelash extensions, it is advised to research the available places in your area and be aware of any ingredients used. Also, make sure your eyelashes are clean and blepharitis-free prior to applying extensions.

Cosmetic Tattooing

Micropigmentation, permanent makeup, or cosmetic tattooing. These are all different names for the same type of procedure, which may seem great for people who suffer from dry eye because they don’t have to apply it every day, but that may not be the case. Eyeliner tattooing is associated with meibomian gland dysfunction (MGD). Also, mechanical microtrauma from the tattoo gun may lead to acute blepharitis and keratitis.

Anti-Aging

Just like makeup, many lotions and skincare products contain chemicals that can be harmful to the lid, cornea, and conjunctiva if they come in contact with the area surrounding the eye. The topical cream Argireline, which you may know as “Botox in a jar” is marketed as a less expensive, less invasive alternative to Neurotoxin injections; however, this product can interfere with MGD treatment as it relaxes the muscle releasing the oil from the meibomian glands, and also contains preservatives.

Application & Removal

I’m sorry, this will not be a tutorial on how to achieve that perfect contour or winged eyeliner. However, this information is just as important as you want to make sure your eyes and eyelids stay healthy while you are showing off your makeup skills!


When applying makeup such as eyeliner and eyeshadow it is recommended to stay outside of the lash line to avoid blocking to meibomian glands. As we know, blocked meibomian glands can lead to styes/chalazia appearing or serious infections. Also, try to apply your mascara to the tip of your lashes only instead of starting from the roots.


Proper makeup removal is an essential step to your beauty routine. If you are a contact lens wearer, it is best to remove your contact lenses before removing your makeup to avoid any particles getting into the tear film and cause contact lens discomfort. Everyone should absolutely remove their makeup at the end of the day before going to bed to, again, prevent clogging your meibomian glands and prevent infection. Avoid makeup removal products that contain oil, parabens, sodium lauryl sulfate, and diazolidinyl urea. It is always recommended to use a gel-based makeup removal. After that step, using a gentle eyelid scrub can help remove excess makeup.

Tips & Tricks

• Many people forget this important step: wash your brushes properly and frequently! You must keep your makeup applicators clean and use a brush cover when not in use. Also, replace your brushes every 6-12 months or immediately if you have developed an eye infection.
• Always use a new applicator at the store when shopping for new products and avoid sharing with others.
• “Hypoallergenic,” “all natural,” “organic,” or “ophthalmologist-tested” does not automatically mean it’s good for your eyes. Read all ingredients.
• Lastly, don’t pump your mascara wand in and out of the container as this action introduces air and bacteria in the bottle. Simply, twirl it around.

What to Avoid

• Ingredients such as the following tend to cause allergies or irritation: arsenic, beryllium, cadmium, carmine, lead, nickel, selenium, and thallium.
• Two-step mascaras consisting of a nylon base coat and a colored top coat.
• Makeup with a higher water content, such as liquid makeup as they are more prone to bacteria and fundus buildup.
• Glitter and powder-based shadows have particles that can get into the eye and cause irritation and/or infections. Cream shadows are a better alternative.

Should I just give up makeup altogether?

Absolutely not (unless you want to). At Boutique Eye Orlando, we are not against makeup. We would just like everyone to be aware of all the possible side effects and harsh chemicals to try to avoid. Makeup is an awesome way to express yourself and show off your style, but eye health should always come first!

Why You Should Avoid Artificial Tears Containing Preservatives

          So, your doctor diagnosed you with Dry Eye Syndrome or Ocular Surface Disease. Now what? Dry eye symptoms such as irritation, redness, itchiness, grittiness, and excessive tearing can be very debilitating. In order to experience some relief, your eye doctor likely recommended the use of artificial tears. However, there are many kinds of artificial tears; and whether they contain preservatives or not could actually make a huge difference.

          Preservatives are essential these days in order to prolong the shelf-life of food, cosmetics, medication, as well as tears. This allows us to store our artificial tears for longer. Although you may feel as though you are saving money due to not having to replace your artificial tears often, it could end up costing much more than a few dollars. The most commonly used preservative in eye drops is called benzalkonium chloride (BAK). This preservative can induce corneal and conjunctival epithelial cell death, damage to the corneal nerves, delay corneal wound healing, interfere with tear film stability, and cause loss of goblet cells. To prevent these adverse reactions, you should not use preserved artificial tears more than four times daily.

          Therefore, the far better alternative is to use preservative-free (PF) artificial tears. You will know if an artificial tear is PF when you see it is individually packaged in small vials. Once you open one vial, it is good for 24 hours and should be discarded after that. People with severe dry eye love using PF artificial tears because there is no limit to how many times a day they should be used.

Redness Relief Eye Drops

With chronic dry eye, the irritation on the surface of the eye can lead to dilation of the tiny vessels in the conjunctiva (the white part of the eye). In this case, the root of the problem is dry eye itself. This is why it is recommended to use preservative free artificial tears regularly and avoid redness-relief eye drops.  Some of the OTC eye drops that provide a quick redness relief should not be used long-term as they restrict those tiny vessels, making the redness disappear, but eventually, it could cause rebound redness, making the symptoms worse when you stop using it.

What is Blepharitis?

Blepharitis is a chronic, inflammatory disease of the eyelids which is caused by an overgrowth of normal bacteria living along the eyelid and the base of the eyelashes. As we age, we make fewer natural antibodies in our tears. For this reason, the prevalence increases with age, even though anybody can get blepharitis at any age.

            This bacteria overgrowth produces a biofilm that traps scurf and debris along the eyelashes. Then, the low-grade inflammation is caused which adversely affects the tear glands, turning down tear production. More bacteria accumulate and a vicious cycle is established, leading to chronic irritation and dry eye.

            Since the eyelids are difficult to clean at home, this bacteria overgrowth can accumulate over the years and the inflammation can cause significant damage to the eyelids and tear glands.

            Thanks to BlephEx, your eye doctor can precisely and safely clean and exfoliate your eyelids and lashes. By doing so, your symptoms will significantly improve, and it will help you avoid the long-term inflammatory damage to your tear glands, preventing chronic dry eye syndrome.

How does it work?

            BlephEx is a painless in-office procedure performed by your eyecare professional. A medical grade micro-sponge is used to precisely and carefully spin along the edge of your lids and lashes. Thus, removing the scurf and debris while exfoliating your eyelids.

            The micro-sponge is disposable, and a clean one is used for each individual, then it is disposed. Afterward, the eyes are rinsed thoroughly.

            The procedure lasts approximately six minutes and is well-tolerated. Most patients describe it as a tickling sensation. Prior to the procedure, a numbing eye drop will be instilled for increased comfort.

What is a Chalazion/Stye?

What is a Chalazion/Stye?

A chalazion, also called a stye, is an inflammatory swelling from a blocked oil gland on your eyelid. These oil glands are responsible for producing the oil that lubricates the surface of your eye. Although they are very annoying and not visually appealing, the good news is that most chalazia clear within a few weeks. However, every now and then you may come across some that want to stay with you a little longer. There are several treatment options available depending on the severity of your chalazion.

Home Treatment Options

If your chalazion just popped up recently, there are some things you can try at home. First and foremost, you want to make sure your eyelids are clean. There are several different lid cleansers available sold over-the-counter. You can try the individually packaged lid scrubs or the bottle with the cleansing soap. These are safe for your eyes. Baby shampoo is also another popular option to clean your eyelids because it won’t irritate your eyes either. Then you can get a warm compress on your eyes and gently massage your eyelids for five minutes. If you have tried these two things twice a day for a week or two and there is still no improvement on your chalazion, then you may consider some office treatments.

Office Treatment Options

Your doctor will evaluate your chalazion and decide what is the best treatment option for you taking the size and position into consideration. It may be injected with steroid, drained, or surgically excised.

Recommendations

To try to prevent a chalazion, it is highly recommended to maintain good eyelid hygiene and to do warm compresses regularly. Make sure you are using some sort of lid scrub or baby shampoo to clean your eyelids twice daily. Warm compresses can help prevent or express clogged oil glands. Also, make sure you are replacing your eye make up as it is marked on each product.

Other Treatments for Blepharitis and Clogged Oil Glands

While there are products sold over-the-counter to maintain proper eyelid hygiene, sometimes it is not enough. If you are someone who suffers from moderate to severe blepharitis, you may want to consider a Blephex Treatment, which professionally scrubs your eyelids clean. Also, if a warm compress with a washcloth is not working, you can purchase an eye mask.

The Effects of Droopy Eyelids on Peripheral Vision

Eyelid surgery is formally known as Blepharoplasty. Most know of this as a beneficial cosmetic procedure. Although most eyelid surgery cases performed today are primarily focused on improving a person’s appearance, Functional Blepharoplasty is done to improve one’s field of vision! Droopy, heavy eyelids may make your face look tired. However, they may also be blocking your peripheral vision. Many people fail to realize there’s more to eyelid surgery than meets the eye.

What is Ptosis?

Those with severe ptosis, a drooping or falling of the upper eyelid, may have developed it from other conditions or simply from aging. Either way, the drooping of the upper eyelid(s) can become so severe that tissue hangs over the lash line and obstructs clear vision. Therefore, eyelid surgery then becomes medically necessary if the eyelid is drooping low enough to impair your vision for driving or other visual tasks. A simple consultation with your oculoplastic surgeon where an observation of the eyelid tissue and positioning is performed, along with a visual field test, can determine if a person is a good candidate for functional blepharoplasty.

Finding the Right Surgeon

Dry Eye vs. Allergies

and what can I do about it?

The two most commonly experienced eye-related problems (not including needing glasses or contact lenses) are dry eyes and ocular allergies. Although some symptoms are similar, there are distinct differences between the two eye conditions. In fact, dry eye and ocular allergy can occur simultaneously and both have different treatment options.  

Dry Eye

Tears are not made of just water. There are numerous components to tears, but think of them simply as having three layers – mucin, water and lipids. A dry eye situation occurs when either too little water/mucin is produced, or if too little lipid is produced. The lipid layer is the outer layer of the tears and its primary role is to prevent the tears from evaporating or spilling over the lid margins. The lipid layer is produced by glands on the edge of the lids called meibomian glands. The majority of dry eye is caused by a decrease in this lipid layer. Certain medical conditions can also cause dry eye. The most common symptoms include burning, sandy, gritty feeling, redness, and sometimes reflex tearing.

Allergies

While eye allergies can also cause redness and tearing, the main symptom is itching. An ocular allergy is caused by sensitivity to a substance that is not usually harmful. When the allergen interacts with cells called mast cells, a substance called histamine is released which causes itching, redness, and swelling. Most allergies are due to environmental factors like pollen, cat dander, dust mites, etc. There are also more serious ocular allergies that require medical intervention.

Treatment & Prevention

Treatment is different for dry eye and ocular allergies. Dry eye treatment includes treating the meibomian glands, the underlying inflammation, and using tear lubricants. The treatment for ocular allergy includes using antihistamine/mast cell stabilizers (to prevent the release of histamine from the mast cells), artificial lubricants, cool compresses and avoidance of the allergen (if possible).

Trying to determine yourself if you suffer from dry eyes or ocular allergies can be challenging. For this reason, we recommend seeing your ophthalmologist for a thorough evaluation. If you do suffer from chronic dry eye, you can set up an appointment for our dry eye clinic to determine the best treatment plan for your eyes.

If you believe you suffer from allergies and you have tried everything in the book to help with your symptoms, it may be time to schedule an allergy test. The allergy diagnostic test contains a panel of 60 allergens that are specific to each region of the country and provides results within 10-15 minutes. We can help you identify the offending allergens and aid in developing a treatment plan.

Many people use over- the- counter products to self- treat dry eye and ocular allergy problems. It is estimated that the cost of doing that exceeds the cost of prescription products which are more effective. IDignity Charity Fundraiser Gala. This Fundraiser was founded by one of Dr. Haas retired partner Dr. Bates. This year our practice supported the event. Our very own executive technician Miguel attended this event this past weekend. IDignity Orlando has hosted monthly client services events where governmental partners and more than 120 volunteers serve over 200 people in need. At these events, qualified clients can get a Florida birth certificate, as well as a Florida ID (Or license renewal). They also offer clients help with getting a Social Security card or verification information. The documents IDignity obtains for its clients give them the freedom to apply for work or school, gain access to shelters, seek help from social service programs, open a bank account or cash a check, secure permanent housing, and overcome many other obstacles to being self-sufficient . IDignity’s purpose is to empower the community and cultivate a more inclusive, productive and secure society. IDignity Orlando has also expanded through satellite organizations in Osceola, Seminole, and Volusia counties. These satellite stations hold quarterly client service events.

DST (Double Suture Technique)

Dr. Haas performs the famous DST procedure developed in Japan, to create a better defined eyelid fold in Asian patients.

What is DST?

DST is a technique that was invented in 2001 by Dr. Akihiro Minami, a renowned plastic surgeon in Japan. Designed to be a minimally invasive procedure, DST does not require significant incisions to be made, hence does not leave behind significant scars on the patient’s eyelids.

As such, DST has become quite a common technique that is used in Japan since its invention. A relatively simple procedure, it can take 30 to 40 minutes for both eyes.

How it DST performed?

Dr. Haas first makes 3 small punctures on the upper eyelid of each eye. Then, using a specific twisting technique, two sutures are passed through those punctures and twisted around each other four to five times. After minimal suturing, the double sutures are tied together to create a fold on the upper eyelid. The result of this procedure is that when the patient opens his or her eyes, the underbelly of the skin is pulled inwards, resulting in a natural looking crease.

Although a quick and often less painful method to obtaining double eyelids, non-incisional procedures are said to be less effective when compared to incisional ones, owing to its high suture suture breakage rate and results that can fade over time. However, studies have shown DST to be both a safe and durable procedure, with results lasting about five years or more. Besides that, the results are also reversible in case patients change their minds.

Is DST for everyone?

Although it can benefit everyone, it is however more suited to certain candidates, in particular, the younger generation who are below 40s.

On the other hand, DST is not suitable for those with problematic eyelids such as those with asymmetrical or sagging eyelids and/or a lot of fatty tissues around the eyelids. Candidates with these issue are usually advised to opt for the conventional blepharoplasty for the best results.

What is Eyelid Surgery?

Blepharoplasty is a type of surgery that repairs droopy eyelids and may involve removing excess skin, muscle and fat. As you age, your eyelids stretch, and the muscles supporting them weaken. As a result, excess fat may gather above and below your eyelids. This can cause sagging eyebrows, droopy upper lids and bags under your eyes.

Cataract. We’re sure we have all heard this word at one point or another, but what is it? And what can be done about it? A cataract is opacity of the lens in our eyes. When this opacity becomes increasingly dense, the transmission of light (creating crisp, clear vision) is obstructed. Sometimes the cataract is so small, no symptoms are noticed. Some people may hear cataracts during an eye examination and think only negative thoughts. Dr. Brian Haas and Boutique Eye Orlando love Halloween! Don’t be surprised to see your favorite Orlando eye doctor wearing a costume on Halloween. But while Halloween means candy and cooler weather, we want to talk a little about eye safety so you can enjoy your Orlando trick-or-treat fest.

 

Contact Lenses– Everyone loves those funky colored and weird shaped contact lenses. But PLEASE do not buy them online or from some random store. Not only is it illegal, it is dangerous! Our optometrist, Dr. Nguyen can fit you with cosmetic contact lenses. They need to be measured and the right shape and fit for your eye. Improper use of cosmetic contact lenses can result in serious eye infections, pain, or even blindness. Some contacts imported into the USA are made with dangerous materials. Luckily there are legitimate cosmetic contact lenses that are fun and safe. But they must be prescribed by your eye doctor. Contact our office for an appointment with Dr. Nguyen.

 

Make-up– Try to use hypoallergenic makeup. Check the ingredients, especially when using around the eye. Some Halloween make-up can irritate the eyes. try and keep all makeup away from the water line/eyelashes.

 

Pointy Props- Avoid pointy props with young children. Plastic arrows and spears might be fun, until you get hit in the eye with them. Avoid anything too sharp, this goes for facemasks too.

 

Costumes That Block Vision– Make sure that your costume allows good vision. Wigs, facemasks, and hoods can block peripheral vision that can make it hard to see cars.

 

Flashlights– Use a flash light or other illuminating device so that you can clearly see where you are going. It’s easy to trip over rocks or uneven sidewalks when it’s dark out.

 

Be Seen– Make sure other people can see you! Wear reflective clothing, glow sticks, and flashlights so drivers can see you. Light color choices help too.

Overview of Chalazion

Are you noticing lumps in or along your eyelids? There is a good chance you may be experiencing chalazion. Similar to a stye, chalazion occurs when a firm lump or cyst in the eyelid forms due to the oil gland becoming blocked. In some cases, a stye may develop into chalazion.

Some of the symptoms of chalazion may be:

  • A small red bump or pimple that grows slowly, but eventually becomes large enough to interfere with your vision
  • Inflammation and swelling around the eye

 

Treatment

Most chalazion need treatment. Make sure not to squeeze or open the chalazion on your own during this time but do  the following method to encourage healing:

  • Apply a warm, wet compress. Doing this three to six times a day can help chalazion heal quicker, and open the blocked pore to begin healing
  • Avoid wearing makeup or contact lenses until the chalazion has healed

In some cases it may require a steroid ointment or a procedure to remove the chalazion. If the chalazion does not get better, please call 407-841-1490!

The best way to treat chalazion is with prevention. Following a few simple suggestions will help to avoid the annoying inconvenience of chalazion.

  • Avoid rubbing your eyes. This can let bacteria in that can eventually lead to the formation of chalazion. If you must rub your eyes, wash your hands first.
  • Replace makeup, especially mascara, at least every six months. Bacteria can grow in makeup.
  • Treat any irritation of infection promptly

The difference between premium and regular cataract surgery is the type of intraocular lens placed into the eye. When it comes to cataract surgery, there are many difference lens choices.
